Use this tab to enter Special Education and Related Services that are planned for the student. Entries on this tab are for section 21 of the IEP form. Later in the process (Implement Plan stage), you can request providers for services identified on this tab. Providers can then log their visits.
To provision Services in an IEP, do the following:
After the IEP is initiated, select the Services tab.
There are two list boxes:
Special Education and Related Services
Supplementary
Aids and Services
Under the appropriate list box,
click Add. If
a Service
exists,
you can edit it by highlighting it and clicking
Edit.
Enter the following:
Service
Projected Start Date and Projected End Date. If a new Service is being added that is identical to an existing service, for example to change the frequency of visits, the Projected Start Date must be after the Projected End Date of the previous Service.
Frequency - The frequency of service, e.g. "n minutes / day," "daily," "n minutes / week," etc. For Supplementary Aids and Services, you can opt to enter free-text to describe the frequency of service.
Location - Select the environment in which the services will be administered. This can be General Education, Special Education or a combination of General Education and SPED.
Extended School Year
- Select Yes
if it is determined that service must extend beyond the end of
the regular school year. An ESY box displays below, where you
can click Add to notate ESY Start Date,
ESY End Date and Comments.
When appropriate, create
an ESY Report. (This is not applicable for Supplementary
Aids and Services.)
Click OK.
The service is now listed on the Services
tab. You may add more services, as necessary.
NOTE: Inactivating and activating services apply when you
revise an IEP (e.g., when
you are revising the Initial,
Annual or Reevaluation).
Once the IEP is given to the parent, you cannot delete a service because
the IEP must reflect in its history that the service was once provisioned.
When the student no longer requires the provisioned service, you may
inactivate it.
Use caution when inactivating a service. If a service is inactive
when you activate an IEP, that service becomes unusable for future
New Revisions. If you inactivated a service in error, please do the
following:
If are in a New Revision and cannot activate the service, delete the New Revision.
Find the active IEP and open it.
Create a New Version.
Go to the Services
tab and activate the service.
For Clarification
of Services and Supports, if necessary, enter additional information
about the proposed services.
NOTE: The Service Availability
Date was previously used in the calculation of the 60-day timeline.
The date will still be displayed for IEP's that were already
created and activated with a Service Availability Date, but the date
is no longer displayed on newly activated IEP's. The only exception
is when a new version is made from an IEP that was previously activated
with a Service Availability Date.
For services added to this tab under Special
Education and Related Services, the provider is assigned on
the Referral > IEP (instructions
below). However, for Supplementary
Aids and Services, the provider is assigned on the Student Profile >
Supports tab.
