Suspect Print Form

The Print Form button on the Suspect Form opens a modal window that allows you to print and manage suspect forms.

 

To print a form:

  1. Search for a student then select the Incidents tab.  

  2. Highlight the incident, then click Open Suspect Form.

  3. Click Print Form. The Print modal opens.

  4. Use the drop-down list to select a form.

  5. Click Print to display the form as a PDF.

 

Having the Form Signed and Uploaded into eCSSS

 

Once the form is signed, it can be entered into eCSSS to become part of the student's electronic record. The instructions below describe this process for both those with and without electronic signature capability.

2. Get signature
 

      1. Page 2 of the PDF, click on the Signature field.

      2. Doing so brings up the Sign-It for Acrobat window. Enter First Name and Last Name, which are required, then click Sign.

      3. Sign the form using the signature pad that is plugged into the workstation. If the signature needs to be redone, you can click Clear.

      4. After the signature is complete, click OK.

      5. Go back to the PDF, and from Page 1 (the coversheet), click Submit.

      6. The Upload Form window is displayed. Click Save then Close.

      7. The signed form is now uploaded to the student's record and listed under Form Name.



3. Scan and upload signed form - If the school will sign the hard-copy form (instead of signing it electronically, as described in above), the hard-copy may be scanned and uploaded into eCSSS. NOTE: Only users with the e-signature right see the Upload button. Scan/upload is optional.

Please proceed as follows:

    1. Scan Page 1 (coversheet with barcode) and subsequent pages at a minimum of 300 dpi and save the scan as a PDF file.

    2. From the eCSSS Home or Student Profile screen, in the bottom left corner, click Upload Form.

    3. On the Select PDF Form window, click Browse and locate your file.

    4. Click OK.

    5. On the Upload Form window, click Save.

    6. The barcode contains the unique information that ties the student to the uploaded form. By scanning and uploading the PDF, the signed document becomes part of the student's record.
