What is an Assessment Report?

 

If the team suspects that a student's need may be the result of a disability, the team may proceed with an evaluation. In some cases, the team may decide that the evaluation must include an assessment. "Assessment" means the administration of specific tests, instruments, tools, strategies and other materials, in accordance with section 8-56-9, to determine whether a student has a disability. For initial evaluations, an assessment cannot proceed without the written consent of the student's parent(s).

 

An Assessment Report is based on sessions an assessor conducts with a student. During an assessment, the assessor gathers specific information, which can include test scores, behavioral observations and interpretations. This information is entered into eCSSS and becomes part of the student's referral record.

 

You enter an Assessment Report only if you receive a request to do so. Only Student Team members are asked to provide an assessment report (i.e., listed contacts, such as family members, do not provide reports).
