This document is a supplement to the Add a Performance Input instructions.
Provide Performance Input by first selecting a Performance Category. Some categories require that you select a Performance Attribute.
Academic -
Relating to Creativity, Math, Reading, Writing, Learning, Second Language
and Communication. (See definitions for Academic Performance Attributes.)
Behavior -
Relating to the student's behavior in the school environment. This
can relate to aggressiveness, poor attendance, disruptive behavior
and illegal activity. (See definitions for Behavior Performance
Attributes.)
Emotional-Internalizing
Problem - Relating to the student's emotional well-being and
how it affects performance. Examples that may be mentioned include
whether the student is anxious, depressive, withdrawn, fearful, moody,
or whether the student has poor self-esteem.
Family Concerns
- Relating to how the student's family life may affect performance.
Examples can include the death of a family member, loss of a pet,
whether family members have serious physical or mental illness, divorce/separation,
new location, incarceration of family member, substance abuse, physical
or emotional abuse/neglect.
Health - Relating
to the student's physical well-being and how it affects performance.
Examples of health conditions that may be mentioned include asthma,
cancer, diabetes, hygiene, pregnancy, etc.
Physical -
Relating to the student's physical capacity and how it affects performance.
This includes Gross Motor, Fine Motor, Vision and Hearing. (See definitions for Physical
Performance Attributes.)
Social - Relating to the student's social skills. For example, your input may relate to whether the student is shy, withdrawn or alienated.
ACADEMIC PERFORMANCE ATTRIBUTES
When providing Academic input, provide it within the context of the Performance Attributes below. Consider whether the student exhibits any of the following:
Creativity
Strength - Exhibits exceptional talent in theater, fine arts, music, etc.
Math
Strength - Demonstrates exceptional ability in math.
Challenge - Demonstrates difficulty in math.
Reading
Strength - Demonstrates exceptional ability in reading.
Challenge - Demonstrates difficulty in reading.
Writing
Strength - Demonstrates exceptional ability in writing.
Challenge - Demonstrates difficulty in writing.
Learning
Challenge - Difficulty following oral or written directions, work is not completed, unmotivated to do school work, lacks enthusiasm for school or learning, attention difficulty, disorganized, poor work habits/study skills.
Second Language - Identify whether the student is a second language learner and how that affects performance.
Communication - Provide observations that relate only to articulation, receptive and expressive language.
BEHAVIOR
PERFORMANCE ATTRIBUTES
When providing Behavior input,
provide it within the context of the attributes below. Consider whether
the student exhibits any of the following traits:
Aggressive - Aggressive or hostile with peers or adults, fighting, swearing and bullying.
Attendance - Poor attendance, chronic absences and tardies, truancy.
Difficulty Following Rules - Breaks rules; not in compliance with school rules and regulations.
Disruptive - Class disturbances, management problems.
Harassment - Intentionally made verbal derogatory remarks, slurs, jokes, insults or constant teasing that causes feelings of humiliation, threat or embarrassment.
Impulsivity - Easily distracted, restless, acts without thinking.
Overactive - Engages in activity level that is beyond what is appropriate for age/cognitive developmental level and is not related to academically over or under-stimulating environments.
Unethical
- Illegal activities including stealing, vandalism, drug use, juvenile
offenses, criminal gang involvement.

When providing Physical input, consider the following areas:
