NOTE: Performance Input is no longer used but may be displayed on older referrals.
A Performance Input cannot be entered unless a request had been made to a team member. Performance Input becomes the basis for the student's Current Performance, which a key component for all types of referrals
To add a Performance Input, do the following:
Expand GATHER DATA by selecting the + sign.
Under Performance
Author, highlight the item containing your name. The Actions menu selections will change.
Select Open Performance
Input from the menu.
The Performance Information window will open.
The Student Name, Student ID and other demographic information will be displayed.
Enter the Date of the performance input in MMDDYY format. (Tips for entering dates.)
Use the drop-down lists to select a Performance Category, a related Performance Attribute. (See definitions), then whether it is a Strength or a Challenge.
Click Add Performance.
A box displays your selected Performance Category, Attribute, and whether it is a Strength or Challenge. Within the box, enter the following:
Strength or Challenge - Use the drop-down list to select Strength or Challenge.
Description
of Behavior and Evidence - Enter concrete data that will
be used when the team creates need statements later in the process.
Examples include test scores, report card grades, number of discipline
referrals and observations of the student's behavior (frequency,
severity and duration).
NOTE: You can enter a strength
and a challenge under
the same Performance Category
by creating two separate entries.
You may delete items by clicking .
Repeat steps 8 through 10 as necessary.
Click Save then Close.
Your input is included with others in the View All Performance Input screen. In addition, the input becomes the basis for the student's Current Performance. For an overview, see Student Support Process - Quick Guide.
