What is Prior Written Notice?
A Prior Written Notice (PWN) is required by the State to be provided to the parent whenever there is a proposal or refusal to initiate or change the student’s identification, evaluation, placement or program (provision of FAPE) for an IDEA/504 student. Multiple PWN records can be created at different points in the Student Support Process.
A PWN must be created for each change made by the Student Team for Evaluation, Eligibility Determination, implementation of services and revocation of consent by parent/guardian for continued services. As such, the PWN serves as a documentation tool for key actions taken by the team. You must use the PWN to report the following to parents:
Outcome of Conference-Evaluation.
Outcome of Conference-Eligibility.
Provisions in a 504 Plan or Individualized Education Program (IEP).
Set the date used to calculate the next Reevaluation on the Triennial Reevaluation Agreement.
