What is an Action Plan?
The Action Plan addresses the needs of students who require supports or services but are not suspected of having a disability. The AP articulates the student's needs, goals and benchmarks and includes the services required for the student to succeed. When appropriate, use the AP to address requirements in the Comprehensive School Alienation Program and Pregnant/Parenting Teen Program.
Before drafting an AP, the Student Team must take the referral through steps outlined in the Student Support Process (SSP). In particular, the team must complete the student's Current Performance. Needs statements in the Current Performance become action items in the AP. When developing an AP, the team must include either or both of the following, which are tied to each of the plan's need statement :
Benchmarks - Defined by Hawaii Content & Performance Standards III (HCPS III)
General Learner Outcomes (GLOs) - See GLO definitions
Service providers are not required to log supports and services provisioned in APs.
Creating a New AP
To start a new AP, do the following:
If no AP exists OR a previous AP is closed - Log a new referral then follow the necessary steps to develop the new AP (e.g., create Current Performance).
If a previous AP is active - You have the option of creating a New AP Review. This type of AP copies information from the active AP and becomes the basis for the new AP.
Closing an AP
When the plan is no longer necessary, close it by going to the Actions tab and entering an AP Close Date, Reason and additional comments. Once you close an AP, you cannot add another AP in the same referral. If you need to create a new AP, you must first log a new referral. See Starting a New AP below.
Progress Report
The Student Team evaluates the student's progress against AP Benchmarks and documents the evaluation in the AP Progress Report. The report is completed in any of the following circumstances:
At the end of the school year.
When the support end.
When the student transfers to another school.
Supplements
Some services require specific information in addition to what is noted on the AP screen. If a student requires any one of the following Level 3 services, you must enter additional information in supplement screens:
Pregnant/Parenting Teens (PPT).
CSAP: Special Motivation Program.
CSAP: Alternative Learning Center (ALC).
