When a student’s special education services are being terminated because the student graduates or exceeds the eligible age limit, schools are required under IDEA 2004 to provide the student with a Summary of Performance. As the student pursues post-school goals, the summary serves as the documentation needed for the student to access support services.
To create a Summary of Performance, do the following:
Expand PLAN.
Highlight the IEP Active Cycle. The Actions menu will change.
Select New Summary of Performance from the Actions menu,
On the Functional Performance tab, do the following:
Note that the To field contains the student's name as read-only.
Use the To Parent/Guardian drop-down menu to select the person to whom the Summary of Performance will be given. This person must be a listed as a Contact.
For Summary of Performance Date, enter the date the Summary of Performance is created. (Tips for entering dates.)
Enter the Provided Student/Parent Date. If the student has not reached age 18 (the age of majority), the school must provide the SOP to both Parent/Guardian and the student. When printed, the SOP will be addressed to both
Mark whether the services are being terminated because the student Received Diploma or Reached 22.
Enter Summary of Academic Achievement and Functional Performance.
On the Post Secondary Outcomes tab, enter recommendations for agencies or support services that will assist the student. The fields include:
Academic
Vocational
Community Participation
Independent Living
Related Services
Click Save.
To print, click Print
