Transition Services are a coordinated set of activities aimed at helping students reach post-high school goals. As such, the Transition Services portion of the IEP addresses long-range goals that can span several years. Entries on this tab are for sections 11-15 of the IEP form.
To document Transition Services on an IEP, do the following:
After the IEP is initiated, select the Transition Services tab.
Enter the Post High School Goals from the PLEP.
Enter the Student’s Interests.
In the Course of Study box, enter the following:
For Path of Study, select either Certificate or Diploma to indicate the student's intended course of study.
When the student
turns 14 (or younger, if appropriate), enter a statement
describing transition services needed for the student to achieve
post-high school goals.
In the
Transfer of Rights at Age of Majority (18) box, please note
that at least 1 year before the student's
18th birthday, the school must notify the student and parents
that parental rights transfer to the student when he or she turns
18. After this notice is given, enter Notice
Provided to Parent and Student Date.
In
the Transition Services box,
please note that when the student
turns 16 (or younger, if appropriate), enter statements describing
transition services needed for the student to achieve post-high school
goals. Click Add Statement.
Enter the following for each statement:
Post School Outcome - The desired outcome.
Transition Services Needed - Services that will help achieve the outcome
Agency Responsibilities / Linkages - Responsibilities of the agency and how the agency's services are coordinated or linked with other described services.
To delete a statement,
click . Clicking Add
Statement allows you to enter as many statements as appropriate.
