This document provides an overview of incidents. If you do not need an overview, skip this page and proceed with adding an incident.
What is an Incident?
School-related incidents, including misconduct, discipline, school searches and seizures, reporting offenses, police interviews and arrests, and restitution for vandalism are all part of the Hawaii Administrative Rules Title 8 Department of Education Subtitle 2 Education Part 1 Public Schools Chapter 19 regulations. Schools record and track these incidents in the Incident Log.

Incidents are logged and tracked on the School Profile. On the Incidents tab, enter details including, offenses, witnesses, victims, suspects and notification.
Once an incident is saved, eCSSS automatically assigns an Incident Report Number. You
can search for incidents using this number. |

Once a student is named as a suspect, the case becomes part of that person's Student Profile.
Follow-up information on the suspect is logged in the Suspect form. This is also where Manifestation Determination is recorded.

Each Incident record includes relevant information about an incident, including one or more Offense Types. After an Incident is created, you can add to it by recording basic investigation information, noting whether offenses are verified, adding witnesses, victims and suspects, adding disciplinary actions, and sending notifications to parents.
The Incident Form contains the following tabs:
Incident - Record basic Incident Information and a Description of Incident.
Offenses - Identify alleged offenses. Use worksheets to document evidence relating to the named offense.
Witnesses - Identify witnesses and the offenses they allegedly had seen. Record witness statements.
Victims - Identify victims and the alleged offenses under which they have been victimized. Record date parent or guardian is notified. Record victim statements and disposition.
Suspects, and Disciplinary Actions - Identify suspects and possible motivation for the alleged offenses. Record suspect statements and note whether the offenses are suspect or verified.
Notifications - Record various types of notifications related to victim or suspect.
Note that as of eCSSS release 1.12.0, the screens have changed according to new Ch. 19 requirements. Older records can still be accessed and viewed in the previous format.
