Create ESY Report

At the end of an Extended School Year (ESY) service period, providers are required to create a report for services marked for ESY (these services are marked on the IEP-Services tab).

 

NOTE: Multiple providers are able to contribute to this report.

 

To create an ESY Report, do the following:

  1. Search for a student and select the appropriate Referral.

  2. Expand PLAN.

  3. Expand the active IEP. The Actions menu will change.

  4. Under Service, highlight the service for which you would like to create an ESY report.

  5. Select New ESY Report from the Actions menu.

  6. Enter ESY Start Date and ESY End Date in MMDDYY format. (Tips for entering dates.)
     

  7. Enter Provided to Parent Date only when appropriate.

    IMPORTANT NOTE:
    Entering a Provided to Parent Date then saving the report will cause the document to be versioned. (Learn more.)

    Because multiple providers can access and edit the report, use caution when entering this date.
     

  8. Click Add Comments. Two new fields are displayed.

  9. For Provider Name, use the drop-down list to select the provider.

  10. Enter Comments.

  11. Click Save.

  12. To print, click Print Form. This button is displayed only after you click Save.
