This document provides an overview of Consent. If you do not need an overview, skip this page and proceed with completing the appropriate form.
What is Consent?
Before taking certain actions to address a student's needs, the school may need to acquire informed parental consent. The three types of consent forms are:
Consent 102a - For initial evaluations before an Assessment can be conducted.
Consent 102b - For Initial Provisions of Special Education and Related Services or 504 services before a plan can be developed and implemented.
Consent 102c - For IDEA reevaluations before an Assessment can be conducted.
Revocation of Consent 102b - For revoking consent after initially providing consent for Initial Provision of Special Education and Related Services or 504 services.
When a parent/guardian revokes consent for services, this also must be documented.
Required steps before completing a Consent Form
eCSSS will not allow you to create a Consent form until the appropriate Worksheets and PWNs are completed.
To create a Consent
102a - Under DEFINE NEEDS,
you must mark the Evaluation Worksheet
(in the Evaluation Status)
with the decisions shown below (504
or IDEA). In addition, you
must create a corresponding Prior
Written Notice. The consent is created under
the PWN.

102b - Under DEFINE NEEDS,
under Eligibility Determination,
you must create a corresponding Prior
Written Notice. The consent is created under
the PWN.
To create a Consent
102c - You must mark the Evaluation
Worksheet with the decisions shown below (note that Reevaluation
is now selected). In addition, you must create a corresponding Prior Written Notice. The consent
is created under the PWN.
