Use this tab to indicate whether the student will participate in the Hawaii Statewide Assessment.
To document Statewide Assessment on an 504 Plan, do the following:
After the 504 Plan is initiated, select the Statewide Assessment tab.
Select a Statewide Assessment option. Options include:
Will participate in state-wide assessments – Nothing further needs to be done. Continue with other parts of the 504 Plan.
Not applicable for the present school year – Nothing further needs to be done. Continue with other parts of the 504 Plan.
Will participate in state-wide assessments with accommodations – Check any accommodations that will be provided for the student in the Accommodations box. You are required to select at least one.
