Stage 3: Develop Plan

The next stage in the Student Support Process is to Develop Plan.

Based on the student's defined needs, the team develops a plan that consists of measurable goals and appropriate services. For students eligible for IDEA and 504 services, the plan includes the Individualized Education Program (IEP) and 504 Plan, respectively.

Before proceeding with plans, please review some important reminders.

Tasks you may accomplish during these stages are:

504 Plan

• Schedule a Conference.

• Initiate the 504 Plan.

• Create a Prior Written Notice (PWN) - Plan.

• Enter Actual Conference.

IEP

• Schedule Conference.

• Complete the Agreement to Excuse from Attendance form (for IEP Conferences only).

• Develop an Individualized Education Program (IEP) - Includes the following types: Initial, Annual and Reevaluation. Revisions can be made to all types.

• Create a Prior Written Notice (PWN) - Plan.

• Enter Actual Conference.

• Document Consent to Amend IEP

• Summary of Performance
