Initiate a 504 Plan

The 504 Plan is the plan the Student Team develops to address needs of a Section 504 or Chapter 61 student who is eligible for services.  (Learn more.)

To initiate a 504 Plan, do the following:

1. Search for a student and select the appropriate Referral.

2. Highlight PLAN. The Actions menu selections will change.

3. Select New 504 Plan.

4. Proceed as appropriate to various tabs on the 504 Plan:

1. • PLEP

   • Eligibility

   • Accommodations and Related Services

   • Goals and Objectives

   • Statewide Assessment

   • Actions
