About Consent for Release of Information

What is Release of Information?

 

The Hawaii State Department of Education is prohibited by law to release any education records or personally identifiable information without the written consent of the eligible student or parent. The Consent for Release of Information form can be used to gain clearance for DOE to:

 

 

NOTE: Older releases may have Exchange as the Consent Type. This type is no longer used in the current HAR 64 form, but these types indicate that the DOE gained authorization to exchange specified information to and from another named party  (e.g., non-profit organization releasing observation records).

 

You can complete the Consent for Release of Information form and print it through eCSSS.

 

Some schools are able to collect signatures from parents electronically.
