Use this tab to enter Accommodations and Related Services information for the plan.
To view or modify Accommodations and Related Services information, do the following:
After the 504 Plan is initiated, select the Accommodations and Related Services tab.
Under the appropriate list box, click Add. If an item exists, you can edit it by highlighting it and clicking Edit.
For Accommodations, do the following:
In the Accommodation field, enter a description of the accommodation. This accommodation will be employed to support the student's needs.
Enter the Projected Start Date and Projected End Date. (Tips for entering dates.)
Checkmark the Applicable Needs (Concerns) that will be addressed by the stated accommodation.
For Related Services, do the following:
Projected Start Date / Projected End Date - Enter in MMDDYY format.
Enter the Frequency.
Checkmark the Applicable
Needs (Concerns) that will be addressed by the selected
service.
REQUIRED:
When all services and plans
in the 504 Plan are available for the student, you must enter the
Service/Plan Availability Date.
This date must be entered manually.
Doing so will end the 60-Day Timeline
and allow the initial 504 Plan to be activated.
