Suspect CRAFT

The CRAFT tab on the Suspect Form is used to record screening and treatment information for substance abuse.

1. Search for a student then select the Incidents tab.

2. Highlight the incident, then click Open Suspect Form.

3. From the Suspect Form, select the CRAFT tab.

4. Enter the following:

   • Substance Abuse Screening - Enter the date screening was conducted, and select the results of the screening from a drop-down list.

   • Assessment - Indicate whether the student was referred for Substance Abuse Assessment and the date a copy of the assessment was received.

   • Treatment / Counseling - Indicate whether the student was referred for Substance Abuse Treatment / Counseling, and the treatment start and end dates, along with the name of the agency providing the treatment. Enter additional information in the text box provided.

Proceed to enter information on the appropriate tab:

• Incident

• Offenses

• Police

• Investigation (NOTE: Displays for Incidents logged after 6/8/2011)

• Discipline

• Interventions

• MD

• CRAFT

• CAS Appeal

• Supt. Review

• Expedited Due Process

• Attachments
