The Superintendent Appeal tab on the Suspect Form is used to document the review by the Superintendent of disciplinary actions against students.
Search for a student then select the Incidents tab.
Highlight the incident, then click Open Suspect Form.
From the Suspect Form, select the Supt. Appeal tab.
Enter the following:
Review Request - Enter the name of the Appeal Requestor. Indicate the Date and Time the appeal request was received, and enter any Additional Information in the text box provided.
Legal Representative - Enter the name, address and contact information of the representative of the person requesting a review by the Superintendent.
Superintendent Review - Record the Superintendent Decision in the text box provided. Enter the Decision Date and whether the decision is being Appealed, and the Date the Decision Appeal was filed. If there is an exception to the decision, enter that in the Decision Exception Description text box. Enter any Additional Information in the field provided.
Proceed to enter information on the appropriate tab:
