AP-Actions

NOTE ABOUT ACTION PLAN: Before 9/18/2011, schools created an Action Plan for students who were not suspected of having a disability but needed supports and services. This plan is no longer being used by the DOE, but APs are being retained on student records.

 

The information on this page describes the plan for background, but note that the functions for editing the plans have been disabled.

 

Enter dates and other important information on the Actions tab.

To enter Actions, do the following:

  1. After the AP is initiated, select the Actions tab.

  2. Enter AP Date in MMDDYY format. (Tips for entering dates.)

  3. When applicable, enter Filed Date. This is the date you are committing the document to school records (i.e., similar to filing it into a student’s folder).

    IMPORTANT NOTE:
    Entering a Filed Date then saving the Action Plan will cause the document to be versioned. (Learn more.)

  4. As appropriate, mark checkboxes that relate to parent or family involvement.


  5. Enter
    Additional Information
    as appropriate.

  6. Closing the plan - When the plan is no longer needed, enter an AP Closed Date in MMDDYY format and select AP Closure Reason. Once you save the AP with these two fields completed, the AP becomes inactive.

     


    IMPORTANT NOTE:

    Once you close an AP, you cannot edit or create a new version of the AP. In addition, you cannot create another AP in the same referral.
     


  1. When you are done with the AP, click Save.


  2. To print, click Print Form. This button is displayed only after you click Save.


  3. Proceed as appropriate to other areas of the AP:

 

Printing the AP Summary:

Printout includes current year's supports only.

 


Requesting Services: When the AP is complete and after you have added services to the AP, send requests for service to providers.

 


New AP Review: If you need to create a new AP, you may be able to simplify the process through the New AP Review.
