About Eligibility Determination

This document provides an overview of Eligibility Determination. If you do not need an overview, skip this page and proceed with documenting Eligibility Determination.

What is Eligibility Determination?

Eligibility Determination is a comprehensive screen used for a variety of tasks related to whether a student is eligible for 504 or IDEA services. This includes:

• Completing the Eligibility Criteria Worksheet

• Recording decisions (whether the student is eligible for 504 or IDEA services) in the Eligibility Worksheet.

• Completing the Evaluation Summary Report (for IDEA only).

Important Information about Eligibility Determination

• Evaluation Worksheet - Please complete Evaluation Worksheet (in Evaluation Status) before proceeding.

• Activating 504 Plan or IEP - Completing the Eligibility Determination is a key step required before activating a 504 Plan or activating an IEP.

• Evaluation Summary Report - For IDEA, some of the information entered into this screen becomes the Evaluation Summary Report, which is a printed form that is sent to the parent. For 504, there is no such report.

Tabs in Eligibility Determination

The five tabs in the Eligibility Determination screen are:

• Current Performance - Displays Current Performance developed by the team. For IDEA, this is read-only and cannot be edited. For section 504, additional Current Performance information may be added.
   

• Assessment Summary - Displays summary of all assessments provided by team members. This is read-only and cannot be edited.
   

• Evaluation Summary
  
  Decision / Date Fields - Displays decisions made by the team regarding evaluation. This information comes from the Evaluation Worksheet and cannot be edited.
  
  Evaluation Summary Field (for IDEA only) - During the Conference–Eligibility, the Student Team must compose a summary of the student's challenges. This summary is based on previously gathered information, including the Current Performance and the Assessment Summary.

  Eligibility Criteria Worksheet - Worksheets used to guide the team to a decision regarding eligibility.
  
  Eligibility Determination  - This is the team's statement regarding the student's eligibility for services.
