AP Screen Reference

NOTE ABOUT ACTION PLAN: Before 9/18/2011, schools created an Action Plan for students who were not suspected of having a disability but needed supports and services. This plan is no longer being used by the DOE, but APs are being retained on student records.

 

The information on this page describes the plan for background, but note that the functions for editing the plans have been disabled.

 

The guide below provides a quick reference for how to access screens when developing an AP

 

 

Express Concern

 

Log Referral.

Student Profile > Search for Student > Referral Tab > In Actions menu, select New Referral

 

Build Student Team.

Student Profile > Search for Student > Student Team Tab.

 

 

Gather Data

 

 

Schedule Conference – Student Needs.

Highlight GATHER DATA > In Actions menu, select Schedule Conference.

 

Request Performance Input from Student Team.

Highlight GATHER DATA > In Actions menu, select Request Performance Input.

 

Add Performance Input.

Expand GATHER DATA > Highlight appropriate Performance Author > In Actions menu, Open Performance Input.

 

Define Needs

 

Create Current Performance.

Highlight DEFINE NEEDS > In Actions menu, select New Current Performance.

 

Enter Actual Conference.
Expand Referral > highlight appropriate Conference > In Actions menu, select Open Actual Conference.

 

Develop Plan

 

Schedule Conference - AP.

Highlight PLAN > In Actions menu, select Schedule Conference.

 

Develop an AP.

Highlight PLAN > In Actions menu, select New AP.

 

Enter Actual Conference.
Expand Referral > highlight appropriate Conference > In Actions menu, select Open Actual Conference.

 

 

Implement Plan

 

Request service provider.

Student Profile > Search for Student > Referral Tab > Expand PLAN > Expand AP > Select Service > In Actions menu, select Request Service Provider.
 

Complete a Pregnant/Parenting Teens (PPT) supplement, if applicable.

Student Profile > Search for Student > Referral Tab > Expand PLAN > Expand AP > Select Service > In Actions menu, select Open PPT Supplement.

 

Complete a Comprehensive School Alienation Program (CSAP) supplement, if applicable.

Student Profile > Search for Student > Referral Tab > Expand PLAN > Expand AP > Select Service > In Actions menu, select Open CSAP Supplement.

 

Evaluate Plan

 

Send parent AP progress reports.

Student Profile > Search for Student > Referral Tab > Expand PLAN > Highlight appropriate AP > In Actions menu, select New AP Progress Report.

 

Schedule Conference - AP.

Highlight PLAN > In Actions menu, select Schedule Conference.
