Student Support Process

The guide below provides screen-related tasks in the Student Support Process (SSP). It does not detail all procedures required in the process.

Express Concern	All Referrals Log a Referral. Build Student Team Incidents Log an Incident.
Define Needs	All Referrals Schedule Conference Create Current Performance. Enter Actual Conference. IDEA and 504 - Evaluation Create New Evaluation Status and Complete Evaluation Worksheet. Create a Prior Written Notice (PWN) - Evaluation. Complete a Consent 102a form if initial evaluation requires Assessment. Complete a Consent 102c form if reevaluation requires Assessment. Send request for Assessment Report. Submit an Assessment Report. Address Guardian's Revocation of Consent, if applicable. IDEA and 504 - Eligibility Schedule Conference If applicable, record Manifestation Determination in the Suspect form. Document Eligibility Determination. Create a Prior Written Notice (PWN) - Eligibility. Complete a Consent 102b for initial IDEA or initial 504 evaluations. Address Guardian's Revocation of Consent for Continued Services, if applicable. Create a Prior Written Notice (PWN) - Revocation. Incidents Enter Actual Conference - Manifestation Determination Enter Manifestation Determination.
Develop Plan	504 Plan Schedule a Conference. Initiate the 504 Plan Create a Prior Written Notice (PWN) - Plan. Enter Actual Conference. IEP Schedule Conference. Complete the Agreement to Excuse from Attendance form if applicable. Complete Consent to Amend an IEP without a Meeting, if applicable Develop an Individualized Education Program (IEP) Create a Prior Written Notice (PWN) - Plan and Placement. Enter Actual Conference.
Implement Plan	504 Plan Request provider services. Log services. IEP Request service provider. Log services in Service Log IEP Progress Reports. Create ESY Report, if applicable. Create Summary of Performance, if applicable.
Evaluate Plan	All Plans Schedule Conference.
