About Action Plan

NOTE: Before 9/18/2011, schools created an Action Plan for students who were not suspected of having a disability but needed supports and services. This plan is no longer being used by the DOE, but APs are being retained on student records.

What is an Action Plan?

 

The Action Plan addresses the needs of students who require supports or services but are not suspected of having a disability. The AP articulates the student's needs, goals and benchmarks and includes the services required for the student to succeed. When appropriate, use the AP to address requirements in the Comprehensive School Alienation Program and Pregnant/Parenting Teen Program.

 

Before drafting an AP, the Student Team must take the referral through steps outlined in the Student Support Process (SSP). In particular, the team must complete the student's Current Performance. Needs statements in the Current Performance become action items in the AP. When developing an AP, the team must include either or both of the following, which are tied to each of the plan's need statement :

 

Service providers are not required to log supports and services provisioned in APs.

 

Creating a New AP

 

To start a new AP, do the following:

 

Closing an AP

 

When the plan is no longer necessary, close it by going to the Actions tab and entering an AP Close Date, Reason and additional comments. Once you close an AP, you cannot add another AP in the same referral. If you need to create a new AP, you must first log a new referral. See Starting a New AP below.

 

Progress Report

 

The Student Team evaluates the student's progress against AP Benchmarks and documents the evaluation in the AP Progress Report. The report is completed in any of the following circumstances:

Some services require specific information in addition to what is noted on the AP screen. If a student requires any one of the following Level 3 services, you must enter additional information in supplement screens:
